One day, Ami and her friends attend a game show event where they learn about a new otome (young girl) dating game, and meet the idol group named Pandemic 4, whose members introduce themselves as Kunza, Nepha, Jed, and Zoi.

Things are not as they seem. It turns out, as soon as the girls leave, that the four men have recognized them as the sailor guardians, warriors opposed to their Dark Kingdom and their quest for the silver crystal of infinite power called the ginzuishou. One of the girls, they believe, must have this relic. They are told to capture each of them alive by their ruler, Queen Beryl. The sailor guardians aren't slow, either. They have realized that energy is being stolen from young girls, and Ami has pinpointed that it's because of these otome games being put out by the supposed idol group.

They meet the group again by chance, and the men invite them out, each choosing a particular girl. Ami is chosen by Zoi, who likes her short hair and the fact she is composed and intellectual. He asks if they can at least talk, what her dreams are, and what type of guys she likes. (Her type is Einstein.) She continues to give him the brushoff although the girls encourage her to let herself go with it and she finally agrees to spend a little time with him - at the distance of holding onto edges of the same handkerchief instead of taking his hand.

On the outing, once the men reveal themselves as enemies and attack, the girls transform to fight them. Ami, Sailor Mercury, is notably missing from the battle even though she had warned the girls to suspect the men. While the other girls believe there is no danger of Ami being seduced, of all people, they worry about her alone with Zoi.

It turns out the two are playing word games, and even share an umbrella as the weather turns stormy. While Zoi goes to find it, Ami recites pi to several places to calm herself down and notes that this first love somehow feels nostalgic, like something she's forgotten. Zoi feels much the same... Ami consults the dating sim advisor, but it's when a clap of thunder startles her that she throws herself into Zoi's arms.

The rest of the enemies arrive then, hoping to abduct Ami before she can transform. Zoi protects her during the battle between the guardians and the enemy's monsters called lemures, but she is held captive since the girls find their powers weakened. The men finally introduce themselves as the generals of the Dark Kingdom, the shitennou known as Kunzite, Zoisite, Nephrite, and Jadeite. Chiba Mamoru, Usagi's boyfriend who the Dark Kingdom know as Prince Endymion, offers to go with them if they agree not to harm Ami while she is in their custody. Usagi, Sailor Moon, offers to go instead, but he promises he will save Ami and escape.

Within the Dark Kingdom, the shitennou begin to realize something is disturbing and begin to suspect Zoisite. He, for his part, visits Ami in her cell and immediately confesses everything, both who he truly is and the fact that he doesn't want to fight or hurt her. She assures him she feels the same, and the two still call each other by intimate nicknames. Because of their feelings of familiarity, they conclude they are both forgetting something very important, and they decide to try to get their memories back just as the other three shitennou ask Beryl directly about their own confusion, and she reinforces her control over those three.

Ami has a terrible feeling something is wrong, just as events begin to conspire to re-start an ancient war between the Dark Kingdom and the Moon Kingdom. She hears the voice of Queen Serenity as the other senshi visit the ruins of the Moon Kingdom.

We next see a scene of the ancient past, with Princess Mercury, Ami's former self, helping the moon princess Serenity dress for a visit from Earth and the entourage of Prince Endymion. Princess Mercury is thoroughly teased over her feelings for one of Endymion's men, none other than Zoisite, and when the delegation from Earth arrives, Prince Endymion teases Zoisite in the same manner, although the two insist their relationship is only friendly.

There is a grand ball with each girl in the arms of her favorite, Princess Mercury with Zoisite. He takes advantage of the opportunity to warn her that Queen Beryl of Earth has become jealous and suspicious of the Moon and may plan to attack, using and manipulating the shitennou. She hurries off to pass the message to the others, not knowing that as soon as she leaves, the other shitennou confront Zoisite for this treason, and Nephrite kills him with a sword from behind. He dies with Mercury's name on his lips.

It is too late for peace anyway. Earth's forces have invaded the moon. Prince Endymion confronts Queen Beryl, who accuses him of abandoning Earth by protecting Princess Serenity. The other princess soldiers draw their swords to defend her and a melee battle breaks out in the Moon Castle. Prince Endymion is killed defending his love, and in her grief, Serenity takes her own life with his sword. Using the ginzuishou, the queen Serenity's mother entrusts all her hope to the future, and sends the senshi and princess and prince to be reborn.

Back in the present life, everyone has begun to remember this past, including Ami and Zoisite. He decides to believe in her, and takes advantage of Beryl being preoccupied trying to claim Endymion to release her from her cell. Just like in the past, he is confronted by the others, and killed with a sword from behind.

Ami returns as Mercury, transformed. After a brief, inopportune interruption from the dating game advisor, she joins the others in battle and demands to know what's happened to Zoisite. Kunzite informs her, and even tells her Zoisite died calling her name. She answers with a crushing Mercury Aqua Mirage.

During the battle, the captured Mamoru, who has been brainwashed to serve the Dark Kingdom, demands the ginzuishou, which has finally materialized as Sailor Moon's crystalized tear. She realizes she must defeat him, and slays him before giving him a final kiss and words of love, then turning her blade on herself, just as in their tragic past life.

Horrified, and grieved at their failures, the senshi and shitennou both mourn. The shitennou sacrifice themselves as they defeat Queen Beryl to redeem themselves, and the senshi give their lives to revive Sailor Moon. With Endymion's help, and the encouragement of the voices of the departed senshi, she finds the way to unleash the ginzuishou's true power to defeat the evil being that had been behind the Dark Kingdom and its plots. In the process, the senshi are also revived and the group has a tear-filled but happy reunion. They all return to Earth together.
